The arithmetic dilemma when defining thinness, overweight and obesity in stunted populations

Background: Critical cut-off values of BMI-for-age z-scores (BAZ) are used to define “thinness”, “overweight” and “obesity”, but the validity these needs be questioned in populations that shorter or taller than reference. We hypothesized prevalence thinness, overweight, obesity depends on population height performed a random simulation.
 Methods: created virtual child aged 2-10 years with normally distributed expressed as height-for-age (HAZ) weight weight-for-age z-score (WAZ), based WHO growth standards references, correlation r=0.7 between weight. adjusted weight-for-height calculated BAZ.
 Results: BAZ age. In short children (mean HAZ=-2 HAZ=-3), thinness falls less 1% youngest rises up 10% HAZ=-2) 13% HAZ=-3) at age 10 years. The 7% shortest close zero 10. Short young tall older more prone misclassified overweight.
 Conclusions: overweight coexistence being – currently referred “double burden malnutrition” consideration what extent this condition is health issue reflects calculation artefacts. arithmetic dilemma particularly affects populations. suggest abstaining from defining “overweight”, “obesity” by BMI z-scores. Different states under- malnutrition should rather classified direct indirect measures body fat.
